A cashless claim stalls most often for a missing document, not a medical reason.
Your insurance card
The physical card or the e-card on your phone. The policy number on it is what the insurance desk needs to start a claim.
A photo identity proof
Aadhaar, voter ID, driving licence or passport, for the patient — not only for the person accompanying them.
The doctor’s advice note
If a doctor has advised admission or a procedure, bring what they wrote. A pre-authorisation cannot be raised without a clinical reason on paper.
A way to pay the balance
Cashless rarely covers everything. Consumables, some medicines and any amount above your room-rent limit are usually paid by you at discharge.
A note on OPD visits
Most health insurance policies in India cover admission, not an ordinary outpatient consultation. Unless your policy specifically includes OPD benefits, expect to pay the consultation fee at the counter and claim it later if your policy allows.
